Proof of Age

Proof of Age

All participants must submit proof of age before the first season they play for NorWest Soccer.
Proof of age is a club and league requirement for all players.

We accept copies of the following as proof of age: 

• Birth certificates, 

• Health cards, 

• Passports, 

• Driver’s Lic., 

Please scan, or photograph, and email proof of age to poa@norwestsoccer.com.

No POAs are kept by our office. They are all destroyed - in accordance with the privacy act - as soon as they are verified by our office.
